Targeted Case Manager
As a part of the Therapy Without Walls team you will offer services to families on a voluntary and court mandated basis. You will develop and implement effective client centered service plans using clinical and therapeutic methods.
As a Targeted Case Manager you will strive to preserve families by working in partnership with Mental Health Counselor, parents, teachers and other participating individuals who work with client to assess their educational, medical, psychological and social needs as it relates to clients mental health diagnosis.
Other responsibilities of the Case Manager include:
•Engaging in professional development including acceptance of supervision, training, and participation in multi-disciplinary meetings
•Producing timely results and documentation of activities provided to clients
•Respecting the culture, traditions, languages, and practices of the persons served
•Collaborating effectively with community partners, supporting agency sponsored events
•Complying with all Therapy Without Walls policies, procedures, ethical standards, and confidentiality requirements
*Bachelor's degree (Internships in also offered)
*Excellent interpersonal skills
*Ability to multi-task and meet deadlines
*Use of technology in daily work activities
*Ability to exert up to 30 Lbs. of force and/or lifting occasionally
*Valid Florida Driver’s License and Auto Insurance
*Qualified candidates of the Targeted Case Management position are independent and motivated decision makers that have an abundance of passion for children’s welfare and the safety of their families. You must have thorough experience with child development, protection, and welfare issues with knowledge of support services and community resources.
